Cholinesterase is an enzyme that catalyzes the hydrolysis of the neurotransmitter acetylcholine into choline and acetic acid, a reaction necessary to allow a cholinergic neuron to return to its resting state after activation. It is mainly found in the liver, pancreas, and the intestine. This test is useful in the case of organophosphate poisoning which can abolish the activity of cholinesterase by irreversibly binding at its active site. It is also useful in diagnosing severe liver disease (acute hepatitis, cirrhosis, or hepatocellular carcinoma), and inflammatory syndromes. INCREASED levels of the enzyme are observed in hyperlipoproteinemic patients. On the other hand, DECREASED levels are found in prolonged liver disease, impaired protein synthesis, or decreased liver cell mass.
