The term adrenal antibodies refer to several different autoantibodies that are directed against the adrenal cortex. Adrenal antibodies are also known as adrenocorticol autoantibodies (ACA). ACA destroy adrenal tissue, interfering with hormone production, by damaging various adrenal proteins, particularly 21-hydorxy enzymes. Tests for adrenal antibodies are used to diagnose autoimmune adrenal insufficiency. 75-80% of patients with Addison’s disease have adrenal antibodies, and 100% of patients with autoimmune polyglandular syndromes have adrenal antibodies. ACA are also associated with autoimmune thyroid disease, autoimmune hypoparathyroidism, primary ovarian failure, atrophic gastritis, and insulin-dependent diabetes mellitus.
