Non-esterified fatty acids (NEFA), or free fatty acids, are derived from adipose tissue stores and represent a major metabolic fuel for tissues. The accumulation of NEFA in the heart muscle cell (myocyte) and release to the extracellular milieu are considered contributors to the pathogenesis of myocardial injury in a number of cardiovascular disease states. INCREASED levels are seen when levels of blood glucose-raising hormones (catecholamines, corticotropin, growth hormone and glucagon) are high, e.g. during fasting. Also, high concentrations are associated with several conditions that convey a high risk of cardiovascular disease, such as diabetes mellitus, high blood pressure, and dyslipidemia.
